Technical details

The surgical instrument is a tool or device specially designed to perform specific actions and desired effects during a surgical procedure or operation. Over time, several different types of surgical instruments have been invented.

Some surgical instruments are designed for use in general surgery, while others are designed for a specific procedure. Thus, the nomenclature of surgical instruments follows certain patterns, such as the description of the measures it performs (e.g. scalpel), the name of its inventor (Kocher's grip), or a scientific compound related to the type of surgery.

Surgical instruments have been manufactured for a long time and there have been reports that surgeons and doctors in India have been using these instruments for thousands of years. In Greece and Rome, ingenious instruments were developed from bronze, iron and silver.

In the twentieth century, with the growth of surgeries, it was necessary the urgent improvement, expansion and improvement of the arsenal of support of physicians in their surgeries, as well as the most suitable materials for their manufacture.

For many years, the availability of biometals suitable for the manufacture of implants and tools or surgical utensils was scarce. With this, stainless steel was used to manufacture surgical and tool tools. This happened with 316. L versus F 138 for
implants and 420 (with higher or lower carbon content) for tools
and utensils.

Stainless steels for the manufacture of surgical tools and utensils used in medicine and dentistry should then have properties that meet the growing requirements in the manufacture and performance of orthopedics, cardiology, surgery, instrumentation and orthodontics.

In Brazil, stainless steel commonly used for tools or utensils, which require corrosion resistance, has been described in the AISI 303 and AISI 304 standards, in versions with more or less carbon. However, the requirements for uniformity, tolerance and consistency of stainless steels used made the adoption of these types of steels a risk factor for quality. It has therefore become necessary to use a more noble stainless steel.

ASTM develops and publishes technical standards for a wide range of materials, products, systems and services. In the specific case of stainless steel F899 303 (available in Bar 303), in accordance with ASTM F899 "Standard Specification for Stainless Steel for Surgical Instruments", it is today the most suitable material for medical applications. The ASTM F899 standard recommends 15 stainless steels of class 4 (martensitic) and 4 others of class 5 (for precipitation hardening) for different applications. Among all, the most common are grades 420B and 440C, of class 4 and XM 16, of class 5.
